‘Medication’ means anything prescribed or over the counter – even Tylenol or Claritin.

To administer it at school, we must have this form on file with a Physician’s signature.

★ IMPORTANT NOTE: Students may NOT ever carry or self-administer any kind of medication at school.

The form is required annually.

Bring the form to your child’s physician for a signature.

Please turn the form in to your school office once complete.
