If your student needs medication at school – regularly or occasionally – this form is required with a Physician’s signature.

‘Medication’ means anything prescribed or over the counter – even Tylenol or Claritin.

In order to administer it at school, we must have this form on file with a Physician’s signature.

★ IMPORTANT NOTE: Students may NOT ever carry or self-administer any kind of medication at school.

You only need to complete it once. 

Bring the form to your child’s physician for a signature.

Please turn the form into your school office once complete.
