Volunteer & Chaperone Guidelines
Volunteering in the classroom:
Parent volunteers are permitted on campus if they have a negative Tuberculosis test (TB clearance) on file. Please provide the school office personnel with either a test result from your doctor’s office or the school nurse will be available to complete a Risk Assessment Questionnaire.
TB clearance is valid for 4 years.
Chaperoning on a field trip:
If driving your own vehicle:
Completed Driver Form
Copy of non-expired Driver License
Insurance Declaration page; form that shows proof that the driver carry the minimum limits of liability insurance coverage as outlined on the Driver Form.
